Why the smoke escaping was bad:

Because the smoke was made of the energy deep within the island that belonged solely ON the island as the island was the only place on the planet that could support and protect it. If that energy escaped into the real world it would have destroyed it. The smoke was twinned with the personality of a person who desperately wanted to journey off of the island, at any cost, even if it meant taking whatever the energy he had bonded to was. In the end, the personality bonded to the smoke wanted to leave despite the damage the smoke energy would have caused to the outside world.

The nuke exploding outwards and the electromagnetic pocket exploding inwards (see: Swan Station implosion) was the incident. This vast collision of opposing nuclear and electromagnetic forces is also probably what created the exotic matter which time jumped backwards to the far past, at the same point the Losties jumped into the future. The exotic matter was then found underneath the Orchard station site by the Man in Black and his team and The Dharma Initiative etc.

The smoke is harmful off island because it is made of the same energy which the island specifically corks inside of itself to prevent escaping into the outside world. The same energy that Desmond released for a brief period of time that pulls a plane out of the sky and the same which Desmond and Locke release which turns the sky purple and distorts spacetime around Desmond. The same energy which the DI release that begins pulling everything into the inside of the island. It's like a reverse big bang.

Also, The Man in Black intends to DESTROY the island, know doubt something which Jacob suspects he would attempt to do all along, knowing the nature of what lies inside the island's core. Doing this would release the energy I explained and cause the reverse big bang effect.
